Complete the Crossroads MDO Authorization Agreement for Direct Payments form to set up automatic withdrawals from your checking account for monthly MDO tuition and bi-annual supply fees. Return the completed form to Ginny Richards in the MDO office.

| OPTION 2: PayPal Payments | |
- You may select to either make a one-time MDO payment or set up monthly MDO payments.
- After clicking one of the payment buttons below, you will be taken to the secure PayPal transaction site.
- The PayPal site will give you the option to pay by using an existing PayPal account or by using credit/debit cards.
- You will have the option to receive an email receipt of your MDO payment.
- Note: Please enter your child's name and reason for payment in the "message to seller" at the bottom of the PayPal payment page.
